STAMPEDE LICENSE PLATE CAMPAIGN

Do you want to be one of the first people ever to have a Stampede License Plate?!  WFHA is accepting applications for the first 50 individuals interested in kicking off the campaign!  To participate, follow one of the two options below:

Open the form below and complete electronically with Adobe Acrobat, including the credit card payment on the bottom of the page.  Email this form directly to West Fargo Hockey at admin@westfargohockey.com .

OR

Print a copy of the form and submit with credit card information completed or check (payable to NDDOT) and either drop off during regular business hours or mail to:

West Fargo Hockey Association
1122 Prairie Parkway Suite 103
West Fargo, ND 58078

Once we have received 50 applications, we will be able to submit to the ND DOT for approval.  Receipt of the license plates will take 3-6 months after we have been approved by ND DOT (just in time for next season!)    Note:  You do not need to have personalized plates (special words) in order to participate, but we love to see all of the creative ways you can use hockey-related words on your ride!

If you have any questions, please contact us at admin@westfargohockey.com or 701-281-4791.
